Process and Requirements

Please note: The FAD directory is for patient-facing clinical providers. If you are a resident or are not billable for services, you are NOT eligible to be listed in the FAD directory.

How to be added to the FAD directory

  1. Are you "billable for services"?
  2. Are you patient-facing (i.e. do you interact with patients)?
  3. Are you NOT a resident?
  4. If yes to all the above, please complete the next two steps:
    1. Submit the Find-a-Doc request form.
    2. Sign up and attend an upcoming photo session. New official photos are required for all new FAD profiles. ID photos cannot be used.

    How to edit, update, or remove a FAD profile

    • Clinic Directors: If a provider leaves UI Health, email their full name, netid, and termination date to
    • Providers or Clinic Directors: Send FAD edits to Please use the email template below – with the provider’s full name and netid – to describe your request.

      "Dr. Jane Doe (netid) is list as Specialty XYZ, but her specialty is Specialty ABC."
