In the Community with Mile Square

Community is at the heart of what we do at UI Health Mile Square Health Center. We are dedicated to improving the lives and health of underserved communities in Chicago and eliminating health disparities.

Central to this mission is the work we do within our communities, where we connect with patients and provide essential information, resources, and care. Our community partners are also a key part of advancing our goals and promoting health equity in Chicago, Cicero, and Rockford, IL.


In the Community

At Mile Square, we are active in the communities surrounding all of our site locations, including our main location (located in the Medical District), Back of the Yards, Englewood, South Shore, and Auburn Gresham, as well as Cicero and Rockford, IL.

We participate in and host community health and resource events, including back-to-school fairs and senior events. During these events, we provide health information and connect community members to our various programs and services.

Community Partnerships

Mile Square partners with many community organizations in and around our various sites, all of which support us by referring new patients to our clinics and special programs. These partnerships help us to expand our outreach and provide essential care to more people.

Chicago Partners

Chicago Housing Authority

The Chicago Housing Authority supports Mile Square by:

  • Promoting clinic services and locations
  • Referring families for care
  • Providing workshops that clear up myths, address social determinants of health, and promote the development of healthier lifestyles

Westside Community Development Collaboration

Westside Community Development Collaboration supports Mile Square by:

  • Promoting clinic services and locations
  • Referring families for care
  • Providing workshops that clear up myths, address social determinants of health, and promote the development of healthier lifestyles

Chicago Public School

Chicago Public Schools (CPS) supports Mile Square by:

  • Promoting clinic services and locations
  • Referring families for care
  • Providing workshops that clear up myths, address social determinants of health, and promote the development of healthier lifestyles

CPS also supports our school-based health centers, where we provide care to community members.

Greater Auburn Gresham Community Development Corporation

Greater Auburn-Gresham Development Corporation supports Mile Square by:

  • Referring individuals to our clinics and programs, such as our Diabetes Prevention Program
  • Networking with other community organizations
  • Bringing awareness to our program and services through their outreach efforts to individuals and families

Just Roots

Just Roots aids Mile Square in:

  • Providing fresh fruits and vegetables to participants in our Diabetes Prevention Program

Lawrence Hall

Lawrence Hall supports Mile Square by:

  • Providing needed resources, such as information regarding Mile Square services, at engagement events for individuals and families
  • Linking care to homeless populations
  • Decreasing myths about health topics, such as COVID-19 and vaccinations

Major Adams Community Committee

Major Adams Community Committee supports Mile Square by:

  • Providing information to families on how to develop healthier relationships and lifestyles as well as access necessary services to address social determinants of health
  • Offering educational workshops and opportunities, such as HIV/STI testing, drug and alcohol resources, and patient holiday gift-giving events, to give back to the community tailored to youth and young adults

St. Leonard's House

St. Leonard’s House supports Mile Square by:

  • Linking previously incarcerated individuals to healthcare and other needed resources
  • Decreasing myths about health topics such as COVID-19 and vaccinations

Westside Coalition for Seniors

Westside Coalition for Seniors supports Mile Square by:

  • Providing necessary resources and engagement events for the senior and disabled patient population in order to offer linkage to care and decrease myths about health topics, such as COVID-19 and vaccinations

Cicero Partners

Town of Cicero Health Department

Cicero Health Department supports Mile Square by:

  • Providing necessary information to non-English speaking and other families in need regarding how to develop a healthier lifestyle balance and how to access important health services to address social determinants of health

Rockford Partners

Compassionate Appreciation for Recovery in Everyone (CARE) Program

CARE Program CARE supports Mile Square by:

  • Collaborating between Winnebago County Jail, University of Illinois College of Medicine Rockford, and Mile Square Health Center LP Johnson Rockford
  • Helping coordinate same-day or next-day behavioral health and addiction services for justice-involved individuals upon release from the Winnebago County Jail
  • Learn more about the Winnebago County Community Mental Health Board (WCCMHB)

Boone County Health Department Community Outreach Advocacy and Recovery (COAR) Program

COAR, which is a member of the Boone County Behavioral Health Task Force, supports Mile Square by:

  • Providing Medication Assisted Recovery via video telehealth
  • Eliminating transportation barriers for individuals living in more rural areas

Winnebago County Health Department Drug Overdose Prevention Program (DOPP)

DOPP supports Mile Square by:

  • Providing services and resources to assist people in abstaining from substance use

City of Rockford Community Action Low Income Home Energy Assistance Program (LIHEAP)

LIHEAP supports Rockford communities by:

  • Providing energy assistance to residents in eligible households through a benefit directly paid to utility companies

Rosecrance

Rosecrance, a community mental health and addiction organization, supports Rockford communities by:

  • Offering inpatient/residential, intensive outpatient, and outpatient mental health and addiction services

University of Wisconsin Addiction Services

University of Wisconsin Addiction Services — SOLARAS Program offers resources, programs, and services for adults and adolescents designed to:

  • Educate the community about drug and alcohol abuse
  • Provide tools to navigate addiction 

Rockford Alliance Against Sexual Exploitation (RAASE)

RAASE, which is a human trafficking prevention organization, supports Rockford communities by:

  • Educating the community on human trafficking to reduce the number of victims of this crime
  • Providing youth education through their Not A Number Program

Get Connected 815

Get Connected supports Rockford communities by:

  • Aiding in re-entry for justice-involved individuals returning to Winnebago County

The Workforce Connection

The Workforce Connection supports Mile Square by:

  • Linking employers and career seekers in Winnebago, Stephenson, and Boone Counties
