Mile Square Health Center Humboldt Park is a clinical practice of the University of Illinois at Chicago - College of Nursing. We are an academic, nurse-managed health center that provides comprehensive, community-based healthcare with UI Health.
The healthcare providers at Mile Square-Humboldt Park are dedicated to caring for the health and wellness of you, your family, and your community. Our nurse practitioners have a holistic approach to care. This means we treat the "whole" person, not simply symptoms and disease. We do more than just identify and treat a specific illness. Mind and body go together, and we want both to be healthy for you.
We want to know how you are doing, how your family is doing, and how we can help. We encourage you and your family to visit us to discuss any concerns you have about your physical and mental health.

Hours
Monday and Thursday: 8:30 am – 7:30 pm
Tuesday, Wednesday & Friday: 8:30 am – 5 pm
2nd and 4th Saturday of the month: 8:30 am – 12:30 pm (Closed on all other Saturdays)
