GENERAL INFORMATION
Thank you for selecting UI Health Mile Square Health Center as your healthcare provider. Mile Square Health Center provides healthcare services for patients at a variety of life stages. Our goal is to provide the highest quality healthcare for you and your entire family.
Much of what we do is primary care, which means we offer continuous, essential day-to-day healthcare for our patients concerning acute and chronic physical, mental and social health issues. Our clinics are located conveniently throughout Chicago and in nearby Cicero. To accommodate the variety of communities we serve, our staff provides assistance in English or Spanish (translators also are available for other languages). Please click here to find the location nearest you.

TO MAKE AN APPOINTMENT
Please call 312-996-2000 to make an appointment.
Please arrive 15 minutes before your appointment to register. If you are a new patient, please arrive 30 minutes before your scheduled appointment time.
Should you need to cancel, please let us know at least 24 hours in advance.
WALK-IN VISITS
Same-day appointments are available on a limited, first-come-first-served basis, and wait times will vary. Our Urgent Care Center also is available – however, due to demand, there may be times that the Center is no longer accepting new patients.
COST
Mile Square Health Center provides medical care to everyone, regardless of their ability to pay. Mile Square accepts a broad range of payment and insurance plans. Based on their income and family size, many of our patients qualify for low-cost services.
WHAT TO BRING
When you come to Mile Square, with an appointment or for convenience care, please bring the following:
- An insurance card for yourself and anyone else in your family who is receiving care (if you have one)
- A list of all medications you currently are taking, including vitamins and dietary supplements
- If your children are receiving care, please bring their immunization cards
AFTER HOURS AND EMERGENCIES
If you have an emergency at any time, please go to the emergency room at the University of Illinois Hospital & Health Sciences System (1740 W. Taylor St., Chicago, IL 60612, 312-355-4000) or the hospital nearest you.
If you are a regular Mile Square patient with a serious but non-emergency medical need when Mile Square clinics are closed, please call 312-996-2000 and ask for the physician on call.
