Our online request an appointment feature is for patients who would like to schedule a future appointment. Your request will be sent to a UI Health representative who will contact you by phone to assist in scheduling an appointment.
Please have your insurance information available. Insurance verification is required to schedule an appointment for a PSA screening or a Urology visit.
